Mental Health Services


We work with individuals, couples, and families to resolve conflicts, heal relationships, and achieve new levels of awareness—both of ourselves and of others.  Services include:

Personal Health Navigation

People with mental and substance abuse disorders may die decades earlier than the average person — mostly from untreated and preventable chronic illnesses like hypertension, diabetes, obesity, and cardiovascular disease that are aggravated by poor health habits such as inadequate physical activity, poor nutrition, smoking, and substance abuse. Barriers to primary care — coupled with challenges in navigating complex healthcare systems — have been a major obstacle to care.

The solution to this problem lies in integrated care, the systematic coordination of general and behavioral healthcare. Integrating mental health, substance abuse, and primary care services produces the best outcomes and proves the most effective approach to caring for people with multiple healthcare needs.


The Changing Perceptions, Organic Impact of Trauma program is a whole health and wellness approach to trauma recovery designed to:

          * Assist clients with setting personal expectations
          * Introduce both physical and mental health objectives as an approach to accomplishing overall life goals,
          * Help identify personal and meaningful motivation,
          * Support health/wellness self-management

The individual served is supported and encouraged to become the director of his/her health and wellness through identifying incremental and measurable objectives and action steps that make sense to the person and that can be used as a benchmark for future success. Health and wellness are discussed as a process instead of a destination.

Whole health engagement and education for survivors of trauma are the key objectives of the service. This is accomplished by facilitating health dialogues; exploring the multiple choices for health engagement; supporting the individual in overcoming fears and anxiety related to engaging with health care providers and procedures; promoting engagement with health practitioners, including, at a minimum, participating in an annual physical; educating the individual on the organic impact of their trauma and assisting them in the development of a self-representing vocabulary to convey their special needs as a survivor to the physical health care community; and other engagement activities as needed.



Reentry Services

People reintegrating back into their communities from incarceration, often do so with little to no assets waiting for them upon their return. For many, their attachment to their communities has waned during their incarceration, and most are not sure if they will be welcomed back into the worlds they left behind.

Individuals served are supported and encouraged to become the director of his/her health and personal wellness through identifying incremental and measurable objectives and action steps that make sense to the person and that can be used as a benchmark for future success.


          * Provide trauma recovery education, personal health navigation and  mental health services
          * Foster community reconnection as a way to promote safer and more stable communities for everyone.
          * Assist formerly incarcerated people reintegrating back into their communities by coordinating services.
          * Promote systemic change within reintegration management

          * Provide evidence-based best practices that includes an overall whole health approach to trauma recovery.
          * Provide education continuation assistance, along with GED preparation and testing services.
          * Assist with employment services, resume’ preparation and interview preparation information.
